Patient Safety Updates

Nurturing a safety culture, learning from mistakes, preventing harm and working as part of a team are all part of the discipline of safety. To this end, the Royal College of Anaesthetists (RCoA), the Association of Anaesthetists of Great Britain and Ireland (AAGBI) engage with partner organisations to create and disseminate our patient safety agenda.

Patient safety is core to all aspects of the College’s training, education and standards for anaesthesia.

The SALG Patient Safety Updates contain important learning from incidents reported to the National Reporting and Learning System (NRLS). The RCoA and the AAGBI would like to bring these Safety Updates to the attention of as many anaesthetists and their teams as possible. SALG encourages you to discuss the updates at your Morbidity and Mortality (M&M) meetings, and we would also like to hear your feedback on the learning points. Feedback can be sent to the SALG administrator at

The updates are published quarterly and contain data from an earlier three month period. To join the safety network, and receive patient safety updates direct to your inbox, please contact the SALG administrator at

Please note that the Patient Safety Update is normally published on a quarterly basis. During 2017-2018, there was an interruption to the usual publication schedule due to contractual issues outside SALG’s control. These have now been resolved and it is expected that from March 2019 the update will be published quarterly. We apologise for any inconvenience caused.
